Capillaritis

Synonym:

Schamberg’s disease
Progressive pigmented purpuric dermatosis

Capillaritis is an eruption predominantly of the lower extremities.

Punctate non-blanching erythematous macules are seen. There is a tendency to aggregation of these lesions into clusters.

The morphology has been described as "cayenne pepper" spots. Red lesions fade to brown hemosiderin. New red lesions appear in crops.

There may be an associated venous insufficiency.

Treatment:

  • Leg elevation and treatment with a mild glucocorticoid such as 1% hydrocortisone cream appears to be moderately effective in some patients.
  • Systemic therapy has not been shown to be helpful.
